Boca Raton Middle School

Student Orthodontic Scholarship/Award

Objective:

· To encourage students to excel academically

Award:

· Full (24 month) comprehensive orthodontic treatment at Feldon Tendler Orthodontics

· This is fully inclusive of records, treatment and retainers

· Total value: over $5000

Eligibility:

· Student must maintain a 4.0 grade point average and a grade 4 in conduct for the entire 2010-2011 school year.

Registration:

· Students must register online at the Feldon Tendler Orthodontics website (www.ftortho.com) by October 31, 2010.

Selection:

· Those students who meet the outstanding academic achievement outlined earlier must mail or fax a copy of their final report card to the office no later than June 20, 2011.

· On June 27, 2011 a random drawing will select the winner of award. Both, the school and the winner, will be notified by mail.

Additional Information:

· If the winner has already started comprehensive orthodontic treatment with Feldon Tendler Orthodontics then a refund will be issued.
